Early diagnosis of heart failure and effective management in primary care prolongs lives, keeps people out of hospital and improves quality of life. The Heart Foundation has previously collaborated with NPS MedicineWise to develop a range of tools, resources and information.Â
The NPS MedicineWise program on heart failure has been developed in collaboration with the National Heart Foundation of Australia. This program is fully funded by the Australian Government  Department of Health.
